HS Student Scholarship Application

Thank you for your interest in our program and our 2024 Scholarship program.

Please complete the application below to the best of your ability.  Be truthful in each section and provide accurate information.  We aim to announce scholarship finalists during the 2024 First Coast Sports Injury Symposium "Careers in Healthcare" for high school students on Friday, April 5.

The deadline for application submission is March 15, 2024, at 11:59 PM.  All application materials must be received by this date/time.

Along with the demographic information on the application form, students must submit a personal resume and answer one of the following essay questions.  Both items will be submitted at the time of application using the links provided within the application.

• What are your plans in the healthcare field and how do you hope to impact this field?
• How has being a part of your high school athletic or sports medicine program impacted you the most?

An individual link will be sent by the JSMP staff to those identified for letters of recommendation.
