2025 JSMP HS student scholarship

Thank you for your interest in our program and our 2025 Scholarship program.

Please complete the application below to the best of your ability.  Be truthful in each section and provide accurate information.

The deadline for application submission is May 15, 2025, at 11:59 PM.  All application materials must be received by this date/time.

Along with the demographic information on the application form, students must submit a personal resume and answer one of the following essay questions.  Both items will be submitted at the time of application using the links provided within the application.

• What are your future goals in the healthcare field, and how do you envision making a meaningful impact?
• How has being a part of your high school athletic or sports medicine program impacted you the most?

An individual link will be sent by the JSMP staff to those identified for letters of recommendation.
